This proprietary network includes providers selected by QHM clients or patients and meeting certain criteria. Members selecting QHM “Select” Network benefit by having lower deductibles and/or co-insurance or in some cases no deductible or co-insurance.

All U.S. claims should be submitted via standard billing forms utilized in the United States which include UB82/92 form for Hospital billing and HCFA 1500 form for Physician billing.
