Medical Case Management
QHM’s case management provides clients with excellent resources for managing the cost associated with catastrophic cases. QHM works with providers, patients, and our case management nurses.
- Assessment: When the client agrees to case management, the case manager will begin a thorough process of assessing the patient’s diagnosis, prognosis, care needs and procedural outcome desired. This detailed work requires contacting the physician and healthcare team as well as speaking with the patient and family to gather all data about the patient’s case.
- Treatment Planning: Once the assessment is complete, the case manager will develop a treatment plan based on his/her findings and the standard of Care developed by the CMSA (Case Management Society of America). The treatment plan will include desired care and cost effective outcomes and timeframes to successfully meet immediate needs as well as short and long-term care needs for the patient.
- Case Evaluation: Once a plan is established the client will receive a report with an evaluation summary that includes expected outcomes, treatment plan, and cost savings. The case manager will continue to communicate with the providers and patient and is responsible for monitoring the quality of care and services.
