The directory contains all PPO network providers who are participating, according to our records. We have made every effort to ensure the accuracy of the information; however, changes occur daily and may not yet be included in the directory. It is possible that the provider selected may not currently be a participating provider in the network, or that a provider not listed has recently joined our PPO network. Please confirm the provider's participation by calling QHM directly. Additionally, the provider may not offer our contracted rates for all services. Please confirm your benefits by calling your health plan benefit administrator or insurance company. It is our policy to verify the contact and professional information for each provider participating in our PPO network.
